Telemedicine Informed Consent Statement

This telemedicine service is provided in accordance with the Sri Lanka Telemedicine Guidelines (Version 1.0, 2024) issued by the Ministry of Health, Nutrition and Indigenous Medicine and the Sri Lanka Medical Council (SLMC).

By proceeding, I understand and agree that:

• Nature of telemedicine: This is a remote consultation via video/audio. The doctor cannot conduct a physical examination. My care may require an in-person visit if the doctor deems it clinically necessary.
• Technology limitations: The quality of the consultation may be affected by internet connectivity, device capability, or technical failures. In such events I will be contacted to reschedule at no additional charge.
• Doctor verification: All doctors on this platform are registered with the Sri Lanka Medical Council (SLMC) and have been verified. SLMC registration numbers are displayed at the time of booking.
• Emergency situations: Telemedicine is NOT appropriate for medical emergencies. If I experience a life-threatening emergency I will call 1990 (national emergency) or proceed immediately to the nearest hospital emergency department.
• Prescription validity: Prescriptions issued via telemedicine are valid under Sri Lankan law only when issued by an SLMC-registered practitioner via a regulated platform. Controlled substances and certain medications cannot be prescribed via telemedicine per MoH guidelines.
• Data privacy: My personal health information is stored securely and processed in accordance with the Sri Lanka Data Protection Act. Consultation records will be accessible to me and the treating doctor only.
• Right to withdraw: I may withdraw this consent and cancel my appointment at any time before the consultation begins by contacting support. Refund eligibility is subject to the cancellation policy.
• Recording: Consultations are not recorded by default. Any recording requires explicit consent from both parties at the time of the consultation as required by SLMC guidelines.
